Health Insurance programs provide workers with various benefits, including a partial coverage of medical expenses, which can be received by showing the health insurance certificate at a clinic or a hospital. Workers can also receive benefits for work-related injuries and illnesses or childbirth allowances. To receive these benefits, applications must be submitted in the format designated by the health insurance association. This article explains about changes in the application forms of the Japan Health Insurance Association (Kyokai Kenpo) in January 2023.
[1] Purpose of the change
The Japan Health Insurance Association has revised application forms to make it easier for insured workers and employers to understand and fill in the forms and to facilitate swift payment of benefits.
Some of the 17 application forms that have been changed are frequently used, as in the following list.

[2] Caution required around the date of change
Applicants may use the new forms from December 28, 2022. Although the old forms may be accepted past the date, the procedure could take longer. Companies should inform applicable employees who wish to receive benefits quickly about the change.
The new forms are already available for download from the Japan Health Association’s website. Examples of how to fill in the forms are also provided. Please check the changes in the forms you frequently use.
